What are ALT and AST?

ALT (alanine aminotransferase) and AST (aspartate aminotransferase) are enzymes — proteins that help your cells carry out chemical reactions. Both are found in high concentrations inside liver cells. When liver cells are stressed, damaged, or inflamed, they release ALT and AST into the bloodstream, which is why doctors use these two values as a window into how your liver is doing.

They're almost always tested together as part of a Liver Function Test (LFT) panel, alongside other markers like ALP, bilirubin, and albumin.


What's a "normal" range?

Reference ranges vary slightly between labs (which is why your report always lists its own range next to your result), but as a general guide:

  • ALT: roughly 7–56 U/L
  • AST: roughly 8–48 U/L

A result just outside these numbers isn't automatically alarming — labs set ranges to cover about 95% of healthy people, so being a little outside the line doesn't necessarily mean something is wrong. Context matters more than the number alone.


The key difference between ALT and AST

This is the part that trips people up: ALT and AST aren't interchangeable, even though they're usually read together.

  • ALT is mostly liver-specific. It's found predominantly in liver cells, so when it's elevated, the liver is the most likely source.
  • AST is found in the liver, but also in muscle, the heart, kidneys, and red blood cells. That means AST can rise from things that have nothing to do with your liver — an intense workout, a muscle injury, or even a strenuous physical job can push AST up on its own.

That's why a doctor looks at the pattern, not just one number. If ALT and AST are both elevated together, the liver is usually the more likely explanation. If AST is high but ALT is normal, a muscle-related cause becomes more likely.


What can cause elevated ALT or AST?

There's a wide range of possible causes, from very common and manageable to more serious. Some of the most frequent ones include:

  • Non-alcoholic fatty liver disease (NAFLD) — increasingly common and often linked to excess weight, high blood sugar, or high cholesterol
  • Alcohol use — even moderate regular drinking can raise these enzymes
  • Viral hepatitis (A, B, or C)
  • Certain medications and supplements — including some common painkillers, antibiotics, and herbal remedies, especially at high doses
  • Recent intense exercise — mainly affects AST
  • Being overweight or having metabolic syndrome
  • Rarer causes — autoimmune liver disease, bile duct issues, or genetic conditions

Mild elevations (say, up to 2–3 times the upper limit) are common and are frequently linked to fatty liver or lifestyle factors. Very high elevations (5–10 times the upper limit or more) are less common and point toward something more acute, like a viral infection, a drug reaction, or a blockage — this is the kind of result worth prompt medical attention rather than a "watch and wait" approach.


Does a high result mean something is definitely wrong?

Not necessarily — and this is worth saying clearly: a single mildly elevated ALT or AST is common, and it's frequently caused by something temporary or manageable, like a recent workout, alcohol the night before your test, or a short course of medication. Liver enzymes also naturally fluctuate.

What matters more than any single result is the pattern over time and the context — your symptoms, your other lab values, your medication and alcohol use, and your overall health history. That's exactly why doctors rarely act on one isolated number; they look at the trend and the whole picture together.


What should you do if your ALT or AST is high?

A few sensible next steps:

  1. Don't panic over one number. Mild, isolated elevations are common and often resolve on their own once the underlying cause (alcohol, medication, exercise) is addressed.
  2. Repeat the test. Doctors often recommend rechecking liver enzymes after a few weeks to see if the result was a one-off or a real trend.
  3. Share your full picture with a doctor — including any medications, supplements, alcohol intake, and recent illnesses. This context is often what solves the puzzle.
  4. Get it checked promptly if the elevation is significant, or if it comes with symptoms like yellowing skin or eyes, dark urine, unusual tiredness, or abdominal pain.


ALT and AST are useful early signals, not a diagnosis on their own. A high result is a prompt to look closer — at the pattern, the size of the elevation, and what else is going on — not a verdict. If your latest lab report has one of these flagged, the most useful next step is usually a conversation with a doctor who can weigh it against your full history, not a search for worst-case explanations.
